For Freshmen:
- Birth Certificate
- Certificate of Good Moral
- Form 138
For Transferees:
- Transcript of Records
- Certificate of Honorable Dismissal
Enrollment Guide

Visit the Dean's Office for requirement submission and evaluation

Secure Enrollment Form and proceed to the Registrar's Office for subjects to be enrolled and let it be signed by the Dean

Proceed to the Treasurer's Office for assessment

Give a copy of the Enrollment form to the Registrar's Office and to the Dean's Office
